We are seeking a skilled and passionate Remote Area Nurse to lead our Chronic Disease Program in a remote Aboriginal community in the Northern Territory. Work alongside a multidisciplinary team to improve health outcomes and build strong connections with the community.
🔍 Key Responsibilities
- Deliver high-quality chronic disease prevention and treatment in collaboration with a multidisciplinary team.
- Maintain and manage an identified client cohort, ensuring proactive and timely follow-up.
- Lead the Chronic Disease team in conducting adult health checks and referring clients as needed
- Develop and implement health education strategies in partnership with other programs and local stakeholders.
- Ensure client files, recalls, and lists are accurate and up to date.
- Support clear communication of goals and priorities through regular team meetings.
- Act as a mentor and resource for community health workers involved in the program.
Essential Criteria
- Minimum 5 years of broad nursing experience, including 3 years in a remote or primary health care setting.
- Strong understanding of program design and delivery in a primary health care context.
- Excellent clinical assessment, problem-solving, communication, and conflict resolution skills.
- Deep understanding of the social determinants of health affecting Aboriginal and Torres Strait Islander populations.
- Proficiency in relevant health software and effective communication across diverse teams.
- Cultural awareness or willingness to learn and ability to live and work in a remote community.
